Update Personal Information

The Office of the University Registrar maintains your academic record information only.

Other records not maintained by the Office of the University Registrar may require updating by other departments. For example, records involving employment require updating by UC Davis Human Resources.

UC Davis WarnMe

At certain intervals during the year, students are prompted, via SISWeb, to provide or update contact information for the WarnMe system. This information will only be used when WarnMe is activated by campus emergency personnel. Students are not required to submit WarnMe information; however the SISWeb prompt will continue to inform students about this important service. For more information, see UC Davis WarnMe.

Address & Phone Number Update

Continuing & New Students

Update your address using SISWeb. Under the Personal Information tab, select Update Address(es) and Phone(s).

If you no longer have access to SISWeb, you can change your mailing address by completing and submitting the Address Update form to registrar@ucdavis.edu.

Change of Legal Name on University Record

To change your name on all of your university academic records after a legal name change (your student ID number will remain the same), you can complete the Petition for Name Change on University Record webform and upload your supporting legal documentation. You may also use this form for name corrections.

If you don't have an active UC Davis computing services account, you may submit the paper form with your supporting legal documentation to the Office of the University Registrar.

Acceptable legal documents verifying a name change:

  • Birth Certificate (acceptable only for correcting spelling errors).
  • Certificate of Naturalization papers.
  • Certificate of Registered Domestic Partnership.
  • Court Order (Final Judgment of Change of Name).
  • Divorce decree.
  • Driver's license.
  • F-1 or J-1 Student Visa or Permanent Resident card.
  • Marriage license.
  • Passport (Book or Card).
  • Permanent or Conditional Permanent Resident card.
  • Resident Alien card.
  • Uniformed Services Military ID card.

Unacceptable documents:

  • Petition of Name Change.
  • Social Security Card.
  • Petition for Naturalization.
  • Employee ID card.
  • Professional License Card.

To change a married name back to a maiden name, you must provide legal documentation other than a birth certificate.

Student Lived Name

UC Davis recognizes that many students may use a name other than their legal name. Therefore, the university provides students an opportunity to choose to identify themselves with a lived name in addition to their legal name. A student’s lived name is used in all university systems, communications, and reporting, except where the use of the legal name is necessitated by university business or legal requirement. This lived name appears instead of legal name in the UC Davis Campus Directory. As of September 23, 2023, the student information system and other major systems display a student’s lived name. To be compliant with the University of California's Gender Recognition & Lived Name Policy (GRNL), it is expected that all systems accept a lived name by December 31, 2023. Some records, such as paychecks or financial aid, that require the use of a legal name, do not use a lived name.

You may specify a lived name by editing the UC Davis Campus Directory. Updating a lived name in the directory results in this name being propagated to university systems; allow 24-72 hours for it to be displayed in other systems. If a lived name contains inappropriate or offensive language or is being used for misrepresentation, the University reserves the right to remove a lived name.

  • Attempts to use a lived name for the purpose of misrepresentation, avoidance of a legal obligation, or other mal-intent is subject to the appropriate UC conduct policies and/or local, state or federal laws.
  • Due to system limitations, only the following characters can be used: A-Z, a-z, and space, hyphen or dash, period and apostrophe if used with alpha characters.

Be aware that campus systems populating a lived name could include ones for which you have granted access to family or other third party; e.g., MyBill. Furthermore, all communications, including those to your home address, will be addressed to your lived name. By the end of Fall 2023, official academic transcripts and diplomas will allow for the use of either your lived or legal name.

Set Your Lived Name

From the UC Davis Campus Directory, select Update my Davis directory listing. Edit the appropriate name field(s), and then select Submit. You can also update your gender identity on this form; this also propagates to university systems. Once you set a lived name, allow 24-72 hours for it to be displayed in other systems.

Applying Your Lived Name to Your AggieCard

After setting your lived name in the UC Davis Campus Directory, allow 24-72 hours for your lived name to propagate to the Student AggieCard system, and then you request a new AggieCard at the AggieCard Office.

Campus Department Usage of Legal Name

Some campus departments interact with, and send reports to federal, state, and other government agencies that require a legal name. Additionally, certain systems and documents require the use of the legal name and do not display lived name; e.g., payroll, tax documents.

Lived Names on Official Academic Transcripts & Diplomas

Students may select either their lived name or their legal name on official academic transcripts and diploma.

Birth Date Change on University Record

To change your birth date on your university academic record, you must submit the paper form with your supporting legal documentation to the Office of the University Registrar

Social Security Number Change on University Record

A student's Social Security number is used to verify personal identity in the UC Davis Student Records System. In compliance with California state law, Social Security numbers are confidential and are not used as student identifiers.

To submit, change, or correct your Social Security Number you may use the Social Security Change webform and upload a copy of your Social Security Card.

If you don't have an active UC Davis computing services account, you may submit the paper form with a copy of your Social Security card to the Office of the University Registrar.

Citizenship, Immigration & Visa Change

To correct your record, or to notify a Residence Deputy of a recent change to your Visa, Immigration or Citizenship, complete the Change of Citizenship, Immigration & Visa form.
